What Is Regenerative Endodontics?
Regenerative endodontics draws on the body’s healing power to facilitate the regeneration of damaged root canals. The innovative approach enables immature permanent teeth to continue developing their roots and strengthening their walls, promoting natural and healthy tooth growth.
Endodontists have a deep knowledge and understanding of pulp biology, dental trauma, and tissue engineering, which can be applied to biologically based regenerative procedures. They strive to preserve natural teeth, which is always the ultimate goal of endodontic care.
What Is Vital Pulp Therapy?
Vital Pulp Therapy is most often used in children and teens up to about age 17 whose permanent teeth have developing roots and living pulp tissue. When trauma, decay, or infection reaches the pulp chamber, but the root is not fully formed, this technique helps eliminate disease while preserving the tooth’s ability to grow and mature naturally.
The procedure involves:
- Carefully removing any infected or necrotic pulp tissue
- Filling the root canals with a medicated gel, typically containing antibiotics and healing agents to prevent reinfection and stabilize the tooth
- Removing the gel after about a week and gently inducing bleeding at the tip of the root (apex) to promote stem cell activity
- The bleeding forms a natural blood clot, which acts as a scaffold for stem cell growth and nerve and tissue regeneration
- Placing a biocompatible seal, which helps the body regenerate healthy tissue over time
With successful therapy, patients may regain nerve function, including sensitivity to temperature changes. The goal is to preserve the pulp, allow the affected tooth to develop normally, and prevent the need for root canal treatment.
What Is Pulp Capping?
Pulp capping is a form of regenerative endodontics that Dr. Carrillo may recommend when a cavity is close to the nerve. There are two types of pulp capping: direct and indirect.
- Direct Pulp Capping: Direct pulp capping is performed when there is damage close to a tooth’s nerve, usually due to deep decay. Dr. Carrillo will place protective material directly over the exposed pulp to encourage healing and preserve the tooth’s vitality.
- Indirect Pulp Capping: Indirect pulp capping is often recommended when decay comes very close to the tooth’s nerve without exposing it. It involves placing a protective layer to shield the pulp tissue and encourage the surrounding dentin to remineralize and heal.
Both pulp capping methods can help avoid root canal treatment, especially in younger patients, and save the tooth’s nerve rather than removing it.
